To File a Name Change Petition in the United States, follow the steps listed
below and gather the REQUIRED DOCUMENTS:
STEP 1)
Order a Certified Copy of your birth certificate (from County or State
Department of Birth Records). Some states provide Birth Record copy service
online. Photocopy your certified birth record and title this page EXHIBIT
"A" CERTIFIED PHOTOCOPY OF BIRTH CERTIFICATE" -
see "Exhibit A" sample below.
STEP 2)
Prepare a Notification List page titled "EXHIBIT B"
that includes the Names and Addresses of your Creditors (i.e. Banks
for student loans, Credit card companies, or Department Store Credit Cards,etc.)
and the Name and Address of any former spouses - see
"Exhibit B" sample below.
STEP 3)
Prepare a Notification Letter page titled "EXHIBIT C"(to
be mailed out by you) and send to all of the Names listed on the Notification
List in step 2 - see "Exhibit
C " sample below.
STEP 4)
The following required items in Step 4 can be accessed at:

After printing out the above forms, complete the Name
Change Application (Page 1)
and sign and date it.
Complete the Petition for Adult Name Change
(Pages 2-4) with the attached Affidavit
"(EXHIBIT D") (Pages
5-6) and have both documents signed, dated, and notarized.
See sample of Affidavit "Exhibit D" below (Notary Service from
a local bank typically costs about $5.00 per notarization).
Also included with your Name Change Forms is a Court
Order (Page 7). Fill
in your Name and Address Info above "Petitioner." *IMPORTANT*
The remaining parts of this Court Order will be filled in by the Judge
at the time of your hearing. After your court hearing, you will then use
this court order when changing your name on documents such as your
driver's license (see copy process in STEP 6 below).
STEP 5)
Submit a Name Change Notice Ad to be advertised in a local paper (not
required in every state). Typical cost for a thirty word notice may cost
about $20.00. Below is an example of a Name Change Ad:

STEP 6)
PHOTOCOPY (keeps copies for yourself) all
of the documents listed above and file the paperwork (including "Exhibits
A-D") at you local district court with the cashier clerk. The
filing fee at the court building may be in excess
of $140.00 U.S. dollars (DO NOT PAY
FOR THIS!!!). Instead, at the time that you are filing your
paperwork, be sure to request a FILING FEE WAIVER
(which is based on income) from the filing clerk to be included in with
your paperwork. The filing clerk will give you
a case number and a date and time with a judge that is assigned to oversee
your Name Change Hearing. Be sure to record all of this information at
the time of your filing with the cashier clerk!
When you show up for your hearing, ask the judge to please sign your court
filing fee waiver. Otherwise, if you forget - you will get stuck with
paying the full filing fee (in excess of $140.00!) after your court hearing.
COPY PROCESS: DO PAY for at least two copies of your Certified
Court Order that was signed by the judge at your hearing. The copies
can be obtained from the cashier clerk after your court hearing. Make
photocopies of your Certified Court Order
and use these photocopies for your Name Change correspondences. The
fee for Certified Court Order copies
